It should only download new mail, but if you are worried, archive (remove the Inbox label) from the older mail in. On my LAB system, the duplicate certificates was the only issue, the new certificate had been installed correctly to IIS, and to RRAS for VPN Connections. Remove the IMAP account from outlook (export and calendar and contacts in it that you need to keep) then add the account using manual setup.

Sure enough after a few days, my local machine certificate store is full of duplicated certificates. Imports of different library/mail states (migration, avoid duplicates). I noticed this thread in the forum a while ago, and setup a Lab network to reproduce it.
